Initial Comments:
An off-site, state licensure follow-up survey conducted on July 13, 2023 found that Revolutionary Home Health, Inc. had corrected the deficiencies cited under 28 Pa. Code, Part IV, Health Facilities, and Subpart G. Chapter 601. The deficiencies were cited as a result of a state licensure incident investigation survey completed on May 17, 2023.
